- Must have valid drivers license the day of the derby

Minor Participants
- Minor Participants require the following form, which must be notarized.
- Notaries are available the day of the event during check-in – bring your fully filled out, unsigned form with you (must be signed in front of the notary).
- Minors also are required to have a valid drivers license.
Form: PARENTAL CONSENT, RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT
